The .jar needs Fabric Loader and Fabric API installed first — the three steps below. The pack brings both with it and adds the server for you.

Install it

Two minutes. Skip the first two steps if you took the pack.

Install Fabric

Fabric Loader 0.16+ — in the installer, set Minecraft Version to 1.21.1. It defaults to the newest version, which won't work. Then Fabric API for 1.21.1.
Drop the .jar in your mods folder

Make the mods folder yourself if it isn't there yet.
- Windows %appdata%\.minecraft\mods
- macOS ~/Library/Application Support/minecraft/mods
- Linux ~/.minecraft/mods
Add the server

Multiplayer → Add Server → playchameleon.net. No port, no mc. prefix.

Optional. Mod jars set off antivirus scanners now and then — this is how you confirm you got ours and not something else.
